Reporting sick leave

What should you do if you are ill and need to see a doctor?

1. Notify your employer (both Human Resources and your own department) by telephone by 9 a.m. (HR phone no.: 0361 6700-7302) or send an email to the respective official email addresses (personal@fh-erfurt.de and the email of your department)

Please note: After seeing a doctor, please only use the web form or send an email with the required data for the following types of sick leave:

• sick note from a registered doctor/dentist
• sick note after an accident in the workplace
• inpatient stay in hospital

In the case of periods of sick leave

• if you are privately insured
• if you are ill while abroad
• for rehabilitation treatment
• if you are not allowed to work for medical reasons
• if your child is ill
• for reintegration in the workplace after periods of illness

you will continue to receive a sick note in paper form. You must continue to report these periods of absence to your employer (Human Resources and your department) and submit the sick note to Human Resources and Legal Affairs as soon as possible.

2. Notify us of the duration of your sick leave, preferably using the web form or by email to personal@fh-erfurt.de after seeing your doctor, giving the following details:

1. The company number ('Betriebsnummer') of your health insurance
2. Your employee number
3. Your last name
4. Your first name
5. The date your sick leave begins
6. The date your sick leave ends

Please note: Please look up the company number (Betriebsnummer) of your health insurance fund and your employee number (Personalnummer) in advance. You will find the company number on your health insurance company's website, for example. Your employee number can be found on your payslip.
